Job Description

POSITION:                 Hourly Job Coach (PT) (Day and/or Evening)

PROGRAM:               Supported Employment

SITE/BUILDING:       Islip Office Center, Oakdale, NY

                                    Roanoke House, Riverhead, NY

 RESPONSIBILITIES:

• Non-traditional work hours based on consumer need (typically 5-10 hours/week)
• Provide on-site job coaching, skills training, and transportation training to consumers in various workplace locations throughout Suffolk County, NY (Days, Evenings, and Weekends)
• Completion of time sensitive documentation as required by funding source.
• Strong organizational and time management skills
• Proficiency in writing skill and Microsoft Office Suite

START DATE:           As soon as possible

SALARY:       $22.25/hour

Job Qualifications

• High School Diploma or GED
